<p>足底筋膜炎初期最常見的症狀，就是早上起床踩地時，腳跟劇烈疼痛。但活動約半小時</p>
<p>後，疼痛會漸漸緩解，接著在久坐後要站起時，疼痛再次出現。症狀可能時好時壞，嚴重</p>
<p>的時候會有尖銳刺痛感，每一踏一步就痛一下，無法久站也走不遠。</p>
<p><span lang="EN-US">      </span>足底筋膜炎為成人足底疼痛最常見的原因之一，在美國每年就有約<span lang="EN-US">100</span>萬的人因足底筋</p>
<p>膜炎而就醫<span lang="EN-US">[2]</span>，好發年齡平均約為<span lang="EN-US">40-60</span>歲，若是跑者則好發年紀較年輕。足底筋膜炎患</p>
<p>者中約有<span lang="EN-US">10%</span>是跑者﹝<span lang="EN-US">runner</span>﹞，主要是因為跑步時有反覆的腳板動作，足底筋膜努力工作</p>
<p><span lang="EN-US">(</span>見下方文章解釋<span lang="EN-US">)</span>，形成微小創傷<span lang="EN-US">(</span><span class="apple-converted-space"><span lang="EN-US"> </span></span><span lang="EN-US">microtrauma)[3,4]</span>。且大多造成足底筋膜炎，多半與</p>
<p>錯誤的生物力學有關（常見：過度足部內翻，俗稱扁平足。及前足內翻<span lang="EN-US">forefoot</span><span lang="EN-US">varus</span>）<span lang="EN-US">[9]</span></p>

<p><strong><u>首先我們先來看看足底筋膜長甚麼樣</u></strong><strong><u><span lang="EN-US">[1] ??</span></u></strong></p>
<p>足底筋膜(腱膜) 為一厚厚地纖維，顏色為珍珠白，緊密附著在皮膚上的縱向纖維(如圖3)</p>
<p>分成三個部份 :</p>
<p>中間Central portion :最厚，稱為足底腱膜，由內側跟骨粗隆，往腳趾延伸分成五</p>
<p>條，各自連到五個腳趾。</p>

<p><strong><u>足底筋膜在做什麼</u></strong><strong><u><span lang="EN-US">?</span></u></strong></p>
<p><strong>足底筋膜可視為腳底的彈簧，具備 <u>吸震 </u>與 <u>推進 </u>的功能。</strong></p>
<p><strong>正常步態下，腳著地時，足底筋膜可用來吸震；而在腳準備離地時，足底筋膜可用來推進。</strong></p>

<p>本篇將正確步態分成上述兩大功能：吸震 ＆ 推進 ，分別分析：</p>
<p><span lang="EN-US"> </span>第1~3 動作（吸震）:  人體吸震器 <span lang="EN-US">reverse windlass effect</span></p>
<p>當由腳跟落地時，足部做出內旋(足弓往下掉) ，足部關節微微分開(unlocking joint )，使落地</p>
<p>時的體重反作用力被吸收，此步驟為吸震。此時足底筋膜會承受兩倍體重的重量。</p>

<p>第4動作 （推進）: 人體彈簧+ windlass effect(註一)</p>
<p>當走路要準備由大拇指蹬出時，會繃緊足底筋膜（如下圖10）</p>
<p>→足底筋膜將跟骨往腳趾拉</p>
<p>→撐起足弓，把足部關節繃緊(locking joint)，將剛剛吸收的反作用力，釋放成為推進力。</p>
<p>這個機制使推進動作較不費力，直接由剛剛吸收的反作用力轉化為推進力。</p>
<p><span lang="EN-US">p.s </span>所以沒有上述條件的人，需更多的小腿肌幫忙產生推進力，此類的人，小腿會較粗壯</p>
<p>，就是俗稱的蘿蔔腿。</p>

<p>當剛剛上述的關節動作中，若在步態中(第1~3動作)產生過多的人體吸震器<span lang="EN-US">Prolonged</span></p>
<p><span lang="EN-US">reverse windlass effect</span>會導致過多足部內翻，足弓往下掉的動作(subtalar joint</p>
<p>pronation) 延長，下個動作來不及做完（大拇指無法翹起動作），足弓往上升起動作不足</p>
<p>:造成足底筋膜往內下壓過度拉扯，形成足底筋膜發炎[7,8]。另外，若步態中，阿基肌腱</p>
<p>過緊，限制了腳踝往上翹（ankle dorsiflexion range）的動作，也可能因為代償模式使</p>
<p>足底筋膜壓力上升。小腿後方肌腱太緊→腳踝往上翹動作受限→利用往外側偏移來取代翹</p>
<p>起角度不足（因下圖15 mid tarsaljoint 軸心），造成足弓往內掉→ 過度拉扯到足底筋</p>
