<p><strong>關節鬆動術</strong><span lang="EN-US"> :</span></p>
<p>腕部的動作會影響腕隧道的形狀與寬度，當正常屈腕動作，屈肌支持<span lang="EN-US">(</span>隧道地板<span lang="EN-US">)</span>帶會向橈骨靠</p>
<p>近，於是腕隧道近端開口橫斷面積的減小，腕骨中的頭狀骨遠端會壓入腕隧道開口。當極度伸</p>
<p>腕動作時，腕骨中的月狀骨壓向腕隧道，使隧道縮窄。</p>
<p>所以可以利用調整腕骨<span lang="EN-US">(</span>天花板<span lang="EN-US">)</span>的高度<span lang="EN-US">(</span>往上鬆動<span lang="EN-US">)</span>，來達到腕隧道的減壓<span lang="EN-US">(</span>如圖<span lang="EN-US">3)</span>。</p>

<p><strong>        神經鬆動術-</strong><strong>滑動神經(sliding technique)</strong></p>
<p>研究發現腕隧道症候群的患者，正中神經橫向[2,3]和縱向[2,4]滑動是減少的，所以們可以利用神經鬆動術中的滑動技巧，讓受壓迫的神經得到氧氣[1]</p>

<p><u>     儀器治療</u></p>
<p>超音波治療 :將超音波強度調整在0.5-1.5 w/cm2，可誘導改變細胞通透性，稱為“微流(microstreaming)”，能提高組織癒合反應，增加腕隧道內組織的彈性及降低組織沾黏[5]。</p>
<p><u> 肌內效貼布治療</u></p>

										<content:encoded><![CDATA[<p>劉先生是一個診斷為<strong><span style="color: #993300;">坐骨神經痛</span></strong>的病患，患者說：半年前開始右邊屁股、大腿後側和小腿外側都有麻痛現象，痛到後期甚至影響睡眠，半夜常常會被痛醒，且白天無法長時間維持同一姿勢，可以說坐立難安。一般人受廣告影響，以為坐骨神經痛需要&#8221;特效藥&#8221;、&#8221;活絡筋骨藥&#8221;、&#8221;氣功將之打通氣血&#8221;、&#8221;治療坐骨神經&#8221;、&#8221;熱敷電療讓它好&#8221;&#8230;&#8230;種種方法，冷靜冷靜！別好像對付敵人那樣，坐骨神經痛怎麼來就怎麼送走就好，我們特地將坐骨神經痛的物理治療案例分享，讓大眾了解如何透過物理治療快速回復健康狀態。</p>
<h2><strong><span style="color: #008000;">坐骨神經痛的惡性循環</span></strong></h2>
<p>在第一次的物理治療評估，我們透過姿勢觀察、動作分析和理學檢查，很快發現個案已經進入<strong><span style="color: #993300;">惡性循環</span></strong>(如下圖)。最開始因為坐骨神經疼痛產生的避痛動作，使身體歪斜；而身體歪斜會讓肌肉偷懶，不能正確施力；肌肉偷懶久了就萎縮了，而萎縮的肌肉更無法發揮力量，使身體回到好的位置上；最後因長期的不良姿勢，會讓關節壓力變大，神經張力變強，使疼痛加劇。當坐骨神經痛發生時，要做的不是找尋特效藥，改變姿勢就是最佳的良藥。</p>

<p>物理治療師幫患者規劃了徒手治療與運動訓練治療。在每次的物理治療時間，物理治療師會先為患者進行關節鬆動術與神經鬆動術徒手治療，然後再進行運動訓練。<strong><span style="color: #ff0000;">透過治療師專業的雙手，徒手治療能降低神經張力，並使關節回到較好的位置上</span></strong>。再透過運動訓練，讓身體記憶正確的狀態，將失衡的肌肉骨骼回復正常，患者的改善進步很快。</p>

<p><strong><span style="color: #ff0000;">運動訓練能喚醒偷懶的肌肉、強化肌力，使關節穩定、減少神經壓迫</span></strong>。患者在每一次的練習之下，將能逐漸遠離坐骨神經痛的困擾。在這邊我們要特別再次強調，運動訓練治療需要專業物理治療師的設計與指導，千萬別自己看網路影片只看樣子就自己在家做，這樣無法得知你的動作正不正確，如果不正確是加深傷害於無形，曾有患者為了減輕疼痛自己很努力在家做拉筋，結果一個月時間就讓患處越拉越緊，身體的毎個部位都是連動的且有其功能，切記。</p>

<h3><strong><span style="color: #008000;">&lt;姿勢調整與有氧運動&gt;</span></strong></h3>
<p>另外，還需搭配姿勢調整及有氧運動。初期調整姿勢需要時刻提醒自己，因為不良姿勢已經維持太久，身體有了不對的肌肉記憶，只要一不注意就會回到歪斜的位置上。<strong><span style="color: #ff0000;">透過反覆提醒，產生新的肌肉記憶，</span></strong>某天就會發現，即使沒有刻意調整姿勢，也會自然在正確姿勢上，那就往健康邁進一大步囉。</p>
<p><span style="color: #333333;">反之，不良姿勢也是透過反覆，產生的結果，記得不要以為偶爾為之不會變成常常為之喔。</span></p>
<h2><strong><span style="color: #008000;">有氧運動對神經修復很重要</span></strong></h2>
<p><strong><span style="color: #ff0000;">血液就是身體的養分，如果想讓受傷的組織修復更快，就要給它充足的養分</span><span style="color: #ff0000;">。而促進血液循環的最好方法，就是有氧運動。</span></strong>對坐骨神經痛的個案來說，運動要以不誘發症狀為前提，腳踏車或游泳都是很好的選擇，跑步通常較不舒服。</p>
<p>由於個案原本就有良好運動習慣，對於運動的配合度很高，因此進步很快。三次治療之後，已經可以睡過夜，半夜不會再痛醒，且連續行走的時間能超過半小時。目前恢復良好，基本已經不會痛了，只剩腳還會麻，但不影響活動。運動方面可以連續跑六百公尺，正在嘗試慢慢拉長距離，後續希望能打高爾夫球，我們相信這天很快會到來。</p>
<p>結論：當疼痛持續太久，通常會衍生出別的問題，進入惡性循環(拖越久問題越大)。這時不能單單處理病灶處，還要整體評估，將不足的肌力補上，將不當的姿勢調整，才能真正恢復健康！</p>

<div style="clear: both; text-align: left;">經過姿勢評估，物理治療師發現詹小姐呈現<span style="color: blue;"><b>搖擺背(sway back)</b></span>的姿勢(下圖)，<span style="color: blue;"><b>骨盆呈現前傾</b></span>(anterior tilt)且有<b><span style="color: blue;">駝背</span></b>。另外進行神經張力測試，發現個案的左手尺神經呈現陽性反應，<span style="color: blue;"><b>核心肌群與背肌肌力</b></span>較弱。</div>

<div>過去很多人對於物理治療有很多的迷思：物理治療就是熱敷、電療、超音波，這是健保物理治療給大家的觀念，物理治療不只有透過冷熱與電等的治療方式，還有徒手治療與運動治療、衛教指導等；還有一種常見的迷思，就是物理治療相當於西方概念的推拿整復所，不只期待有推拿與按摩，而且收費還跟SPA館一樣依治療時間收費&#8230;&#8230;OMG！物理治療師不是芳療師！</div>
